PRP vs. PRF vs. Exosomes for Hair Loss: Which Is Right for You?
The hair restoration field has moved fast. A decade ago, the conversation was mostly PRP versus surgery. Today, Dr. J Anti-Aging Clinic patients are asking more sophisticated questions: What’s the difference between PRP and PRF? Are exosomes worth adding? What about stem cell protocols and ACell? And which of these actually has the strongest evidence?
Those are exactly the right questions. I’m Dr. J an ABIM board-certified physician with over 25 years of clinical experience and I want to give you a clear, honest answer rather than a menu of upsells.
Power PRP: The Established Foundation
Power PRP (platelet-rich plasma) is the baseline regenerative hair treatment and the most extensively studied. It uses your own blood a simple draw, then centrifuge processing to concentrate platelets. The concentrated PRP is injected across the thinning scalp. Multiple systematic reviews and meta-analyses confirm that PRP improves hair density and thickness in men and women with androgenetic alopecia, with a strong safety profile.
What makes Dr. J Anti-Aging Clinic’s Power PRP different from basic PRP: we use an optimized high-speed centrifuge protocol that achieves a higher platelet concentration than standard processing. More platelets = more growth factors per session.
- Best for: early to moderate androgenetic alopecia, diffuse thinning, telogen effluvium, combining with minoxidil
- Session structure: 3 initial sessions at 4–6 week intervals, then maintenance every 4–6 months
- Evidence level: Multiple systematic reviews and RCTs (PMID: 32410524, PMID: 36264022)
PRF: Second-Generation PRP with Sustained Release
PRF stands for platelet-rich fibrin a second-generation platelet concentrate that uses a slightly different centrifugation protocol (no anticoagulant added, lower speed) to produce a fibrin matrix loaded with platelets and growth factors. The key difference from standard PRP: the fibrin scaffold releases growth factors slowly over days to weeks, rather than immediately.
This sustained release extends the biological effect of the treatment. Instead of a single burst of growth factor activity, the follicle environment receives a prolonged signal. Several clinical comparisons suggest PRF produces results equivalent to or superior to basic PRP, particularly for patients with more advanced thinning.
- Best for: patients wanting extended growth factor release, moderate to moderately advanced thinning, patients who respond partially to PRP and want to build on results
- Can be combined with: ACell, exosomes, LLLT, minoxidil, finasteride
- Evidence level: Growing clinical literature; head-to-head RCTs vs. PRP still limited but outcomes data favorable
Exosome Hair Restoration: The Emerging Frontier
Exosomes are nano-scale extracellular vesicles derived from stem cells essentially messenger particles that carry growth factors, proteins, and RNA signals between cells. When applied to the scalp, exosomes directly activate follicle stem cells, reduce inflammation in the follicle microenvironment, and signal dormant follicles to re-enter the growth phase.
Exosome hair restoration represents one of the most exciting areas of regenerative medicine. The biological rationale is sound: we’re delivering the signaling molecules that drive cellular regeneration without needing to inject whole cells. Early clinical data is promising, showing improvements in hair density and thickness comparable to or exceeding PRP in some patient populations.
- Best for: patients with advanced thinning where PRP alone may have limited reach, patients wanting maximum follicle activation, post-transplant recovery support
- Can be combined with: PRP, PRF, ACell combination protocols amplify results
- Evidence level: Early-stage clinical research; mechanistic rationale well established; larger RCTs still in development
| HONEST EVIDENCE NOTE |
| Exosome therapy is newer and has less published clinical data than PRP. The biological mechanism is well-supported, and early clinical results are encouraging but we don’t yet have the same depth of systematic review and meta-analysis evidence we have for PRP. I discuss this transparently with every patient. Exosome treatment is offered at Dr. J Anti-Aging Clinic as part of a personalized protocol not as a guaranteed upgrade for everyone. |
ACell and Stem Cell Protocols
ACell (now MatriStem) is a porcine-derived extracellular matrix product that provides a biological scaffold for growth factor activity. When combined with PRP or PRF, it creates an environment that amplifies follicle regeneration. Dr. J Anti-Aging Clinic’s stem cell protocols use adipose-derived or bone marrow-derived stem cell preparations to deliver multipotent regenerative signals to the scalp.
These combination approaches are best suited for patients with moderate to significant hair loss who want comprehensive regenerative treatment often in combination with hair transplant procedures or as an alternative for patients not ready for surgery.

How Dr. J Anti-Aging Clinic Builds Your Protocol
At your Dr. J Anti-Aging Clinic consultation, we evaluate your hair loss pattern, stage (Hamilton-Norwood for men, Ludwig for women), scalp health, and goals. We review what you’ve already tried and what’s worked or hasn’t. Then we build a protocol not a menu. For most patients in early to moderate stages, Power PRP is the starting point. For patients with more advanced loss or wanting accelerated results, we layer in PRF, exosomes, and ACell based on clinical indication. Dr. J reviews every protocol directly.
Frequently Asked Questions
Can I get all three PRP, PRF, and exosomes in the same session?
In many cases, yes. Combination same-session protocols amplify the regenerative effect. We call this a stacked protocol PRP or PRF combined with exosomes and sometimes ACell in a single appointment. This approach is particularly effective for patients with moderate to advanced thinning who want maximum results from each session.
Which treatment has the most clinical evidence?
PRP has the most published evidence including multiple systematic reviews and meta-analyses with consistent findings (PMID: 32410524, 36264022, 38789807). PRF has growing evidence with favorable outcomes. Exosomes are newer with strong mechanistic rationale and early promising results. I won’t oversell any treatment’s evidence beyond what’s published.
Is PRF more expensive than PRP?
PRF typically adds cost over basic PRP because of processing protocol differences. The extended growth factor release and clinical outcomes data often justify the difference for the right patient. We discuss specific pricing at consultation.
