One of the most common questions I hear at Dr. J Anti-Aging Clinic is some version of: ‘I’ve heard about the shockwave thing, the PRP shot, and something about Botox for ED. What’s the difference, and which one should I do?’
It’s a fair question, and the honest answer is that these three treatments work through different mechanisms, target different aspects of erectile dysfunction, and aren’t interchangeable. The right choice depends on the specific profile of your ED which is exactly what a physician consultation is designed to determine. But here’s the clinical framework.
The Three Treatments at a Glance
MENZ Wave® Acoustic Wave Therapy (LiSWT)
Low-intensity acoustic waves delivered to penile tissue stimulate neovascularization the formation of new blood vessels and upregulate nitric oxide production. Best supported by the evidence for mild to moderate vasculogenic ED. Works by rebuilding blood vessel infrastructure over time. Results develop over weeks to months. A 2025 meta-analysis of 12 RCTs involving 882 men confirmed statistically significant improvements in IIEF-EF and Erection Hardness Score versus sham therapy.
Think of MENZ Wave® as the infrastructure play: more functional blood vessels, better vascular responsiveness, improved mechanical capacity for erection.
MENZ Shot™ (P-Shot® / PRP)
Platelet-rich plasma drawn from your own blood and injected into the corpus cavernosum releases a concentrated cascade of growth factors VEGF, PDGF, TGF-β that promote angiogenesis, smooth muscle repair, and tissue regeneration. The 2024 Mao meta-analysis of 4 RCTs (413 patients) found statistically significant IIEF improvement versus placebo. The 2024 Andrade review of 17 studies across 1,099 patients supports small but meaningful improvements in most men.
Mao et al., 2024. PMID: 38832665 | Andrade et al., 2024. PMID: 38811395
Think of the MENZ Shot™ as the repair signal: growth factors that accelerate regeneration of the tissue the acoustic waves have stimulated.
MENZ Tox Shot™ Intracavernosal Botulinum Toxin
A small dose of botulinum toxin injected into the corpus cavernosum relaxes the smooth muscle of penile arteries, reducing vascular resistance and allowing improved blood inflow during arousal. Different from PRP not a regenerative therapy in the tissue-rebuilding sense, but a functional relaxation of the vascular impedance that limits erection.
The El-Shaer 2021 RCT 176 men with vasculogenic ED who hadn’t responded to PDE5 inhibitors found significant improvements in IIEF, Erection Hardness Score, and peak systolic velocity on Doppler ultrasound versus placebo. The 2025 Pang meta-analysis confirmed at least 40% response rate in the most comprehensive synthesis of available human evidence.
El-Shaer et al., Andrology, 2021. PMID: 33784020 | Pang et al., 2025. PMID: 40330908
Think of MENZ Tox Shot™ as the vascular release: reducing arterial smooth muscle tone so the plumbing that exists can flow more freely.
How They Complement Each Other
Here’s the clinical insight that most men don’t get from a single-treatment clinic: these three therapies address different parts of the same problem.
MENZ Wave® builds new blood vessels and stimulates nitric oxide production.
MENZ Shot™ delivers growth factors that fuel tissue repair and angiogenesis.
MENZ Tox Shot™ relaxes smooth muscle to reduce vascular resistance in existing vessels.
The combination of wave + PRP has the strongest data: the 2024 SMSNA meta-analysis showed +8.2 IIEF improvement for the combination versus +3.4 for wave alone and +3.2 for PRP alone. Adding botulinum toxin for patients with residual vascular resistance addresses a third variable that neither wave therapy nor PRP directly targets.
Which Is Right for You?
The honest answer: it depends on your specific clinical picture. Here are the general patterns:
Men with mild to moderate vasculogenic ED who are pill-responsive but seeking a longer-term solution: MENZ Wave® is often the starting point, with MENZ Shot™ added for maximum effect.
Men who don’t respond adequately to PDE5 inhibitors: the combination of all three modalities wave, PRP, and botulinum toxin addresses the problem from three complementary angles and has the strongest published rationale for this population.
Men with Peyronie’s disease alongside ED: both MENZ Wave® and MENZ Shot™ have published evidence for Peyronie’s disease in addition to ED, making the combined protocol particularly relevant.
- Dr. J:
I don’t run a single-treatment practice. I run a comprehensive men’s sexual health clinic. That means I can look at your situation, explain what each option does mechanistically, and recommend the approach that fits not the one I happen to offer. Sometimes it’s one treatment. Sometimes it’s all three over a coordinated timeline.
Available in One Place Central Florida
All three MENZ treatments MENZ Wave®, MENZ Shot™, and MENZ Tox Shot™ are available at Dr. J Anti-Aging Clinic in Orlando, under Dr. J’s direct supervision. Men from across the region Dr. Phillips, Winter Park, Lake Mary, Clermont, Melbourne, Tampa access the full protocol without coordinating between multiple providers.
