In medicine, the sum is occasionally worth more than its parts. That’s not usually how things work most combination treatments produce modest additive effects at best. Which is why I paid close attention when the data on combining acoustic wave therapy with platelet-rich plasma injections for erectile dysfunction started showing results that were more than just additive.

The Data That Changed How I Think About ED Treatment
Researchers at Baylor College of Medicine presented a systematic review and meta-analysis at the 2024 Sexual Medicine Society of North America (SMSNA) Annual Scientific Meeting that analyzed 16 studies involving 907 patients. The findings deserve to be stated plainly:
Low-intensity shockwave therapy (LiSWT) alone: +3.4 mean IIEF score improvement vs. control
PRP (P-Shot) alone: +3.2 mean IIEF improvement vs. control
LiSWT + PRP combined: +8.2 mean IIEF improvement vs. control

A combined improvement of +8.2 is more than double what either treatment achieves independently. In the context of erectile function research, where a minimum clinically important difference on the IIEF-EF scale is generally considered to be around 2–5 points, an 8-point improvement represents a clinically meaningful outcome not a marginal one.

What Patients Experience

Who Is the Right Candidate?
The combination protocol is most appropriate for men with mild to moderate vasculogenic ED who want a comprehensive, drug-free restorative approach. It’s also the protocol I discuss with men who didn’t respond as fully as hoped to shockwave therapy alone, and with men who are looking for the strongest non-surgical option available before considering more invasive interventions.
It is not appropriate for everyone. Men with primarily psychogenic ED, severe neurological causes of ED, or certain medical conditions may be better served by other approaches. Part of Dr. J’s consultation is determining where you fall on that spectrum.
